In 1967, Pillay became the first woman of colour to start a law practice in her home province of Natal. She defended anti-apartheid activists (including Nelson Mandela), exposed the state’s use of torture, and successfully fought to secure basic legal rights for political prisoners on Robben Island.
Following the end of apartheid, Nelson Mandela appointed her as the first Black woman to serve as a judge on the High Court of South Africa in 1995.
She served for eight years—including four years as President (1999–2003)—at the International Criminal Tribunal for Rwanda (ICTR). There, she played a monumental role in landmark rulings that officially recognized rape and sexual violence as acts of genocide and crimes against humanity. She later served as an Appeals Chamber judge at the International Criminal Court (ICC) from 2003 to 2008.
