Mass canine vaccination—rather than introducing routine pre-exposure prophylaxis (PrEP) for children—could be the most effective and economically viable strategy to eliminate human rabies deaths.

The breakthrough findings, published in The Lancet Regional Health – Southeast Asia, stem from a ten-year cost-effectiveness study conducted by the University of Glasgow alongside the London School of EconomicsKerala University of Health Sciences, and state health directorates.

Why Childhood Rabies Vaccines Fall Short on Impact

The study evaluated various public health interventions between 2026 and 2035 to determine the fastest path towards zero human rabies deaths in Kerala.

  • High Cost, Minimal Returns: Rolling out routine pre-exposure rabies vaccines for all children in Kerala would cost an estimated additional $10 million (Rs 83+ crore) while preventing fewer than 10 additional human deaths over a decade.
  • Strong Existing Treatment Infrastructure: Because Kerala already boasts strong public access to post-exposure prophylaxis (PEP) after animal bites, adding population-wide childhood immunisation provides negligible incremental protection.
  • Targeted PrEP Exceptions: Researchers noted that childhood PrEP remains valuable only for isolated, high-risk, or remote tribal communities with delayed access to emergency healthcare facilities.

Stopping Transmission at the Source

In contrast, expanding dog vaccination campaigns delivers dramatically higher survival outcomes across whole communities at a fraction of the cost.

The Hidden Unowned Dog Census Problem

A critical hurdle identified by the research team is that Kerala’s unowned street dog population may be drastically higher than official census records indicate.

Evidence suggests the free-roaming dog population could be up to five times larger than reported, bringing the actual total to roughly 2.3 million dogs statewide. Underestimating stray numbers means previous vaccination drives reached far smaller percentages of the animal host reservoir than intended, allowing the virus to persist despite heavy healthcare investments.
